Release of Liability
As the legal parent or guardian, I release and hold harmless Valley Dance and
Fitness, operating as (VDAA), its owners and operators from any and all
liability, claims, demands, and causes of action whatsoever, arising out of
or related to any loss, damage, or injury, including death, that may be
sustained by the participant and/or the undersigned, while in or upon the
premises or any premises under the control and supervision of Valley Dance
and Fitness, its owners and operators or in route to or from any of said
premises.
Medical Emergency
The undersigned gives permission to Valley Dance and Fitness, operating as
(VDAA) , its owners and operators to seek medical treatment for the
participant in the event they are not able to reach a parent or guardian. I
hereby declare any physical/mental problems, restrictions, or condition
and/or declare the participant to be in good physical and mental health. I
request that our doctor/physician ________________ be called and that my
child be transported to ______________________ hospital. Please include
physicians' phone number _______________.
Payment Information
Tuition is due by the 15th the month prior to the month of class. Tuition is
based on 10 months and calculated on 40 weeks of instruction, calculation
dates include recitals, rehearsals and picture days. If accounts are paid
after the 15th of the month, there will be a $15.00 late fee applied to the
account balance. There is a $25.00 returned check charge for any checks
returned by the bank.
Student Handbook
I will refer to the Student Handbook (online) for policies on classroom
etiquette, required dance attire, payments/late charges, cancellation policy
and showcase information. I understand where to find this information or will
ask the office staff for assistance.
Cancelling Registration
I understand that I or my child is registered until I give written notice to
the office which must be received no later than the 15th of the prior month
in which she will not be attending. Unpaid tuition and account balances
remain your responsibility.
Photos and print media
VDAA uses class/event photos for print and/or broadcast media as deemed
appropriate for promotion of VDAA.
